In 1286, Sir Daniel Fortesque and his soldiers fought against the evil sorcerer Zarok, who had raised an army of demons to conquer the world. Sir Daniel was hailed as a hero after the battle, and it is said that he was mortally wounded during the battle and took Zarok with him. However, it seems that the wizard was not defeated in that battle. A hundred years later, he reappears and robs people of their souls to build an army of the undead. Zarok unleashes waves of necromantic energy and inadvertently revives Sir Daniel. The knight, now a skeletal corpse, must right the wrongs of the past and defeat Zarok once and for all.

MediEvil for PS1 is an action game in which the player controls Sir Daniel, explores a 3D environment and battles the many enemies that stand in his way of his ultimate goal. The protagonist starts with his own detachable hand as his primary weapon, but later gains swords and other melee weapons and bows. Money can be collected and spent on items and equipment sold by gargoyle merchants. The hero must also find various rune stones that allow him to access new areas. Each level contains a chalice that can be filled with the souls of defeated enemies. Sir Daniel can only take the chalice after he has defeated enough enemies. Getting the chalice allows access to the Hall of Heroes, where the protagonist can receive new weapons. Collecting all the chalices in the game unlocks a special ending.
